Regarding Data Amendment of Surokkha Apps
Details

For changing or correcting the various information on the Suraksha Apps certificate, such as spelling mistakes, passport, NID, birth registration number or name correction, vaccine name, and date correction, or mobile number correction, an application should be made to the relevant office with the necessary supporting documents.

Mobile Number Correction

1. Copy of vaccine card (if any)

2. Copy of National Identity Card

3. Old Mobile Number (the number with which earlier registered in the "Surokkha" system)

4. New mobile number (currently with the number)
Correction of the name of the vaccine & Date

1. Copy of vaccine card

2. Copy of National Identity Card

3. Certificate of Vaccination Centre
Name, Gender & Date of Birth Correction 

1. Copy of vaccine card

2. Copy of Old NID/Birth Registration/Passport (Photocopy of the document with which registration was done)

2. Copy of New NID/Birth Registration/Passport (Photocopy of the document with which registration was done)
